Signature HealthCARE is hiring an LPN/LVN to provide direct nursing care, including meds and wound care, and to supervise nursing staff. This full‑time role covers Hillcrest & Hermitage Care & Rehab with strong teamwork and patient‑centered care.

The ideal candidate holds an active LPN/LVN license, CPR/BLS, at least one year in clinical nursing (long‑term care preferred), and familiarity with EHR systems and MS Office. Growth opportunities available.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long‑term care experience preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care and skilled nursing procedures.
• Assist residents with activities of daily living and ensure comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during the assigned shift.
• Monitor residents' status and notify physicians of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Coordinate admissions, transfers, and discharges; participate in QA and regulatory activities.
