Job summary

Signature HealthCARE is seeking an LPN for Hillcrest & Hermitage Care & Rehab to provide direct nursing care, assist with daily living activities, and supervise nursing aides. The role includes medication administration, wound care, and ensuring safety and comfort for residents.

The position offers a night shift differential of $8/hour and a $5,000 sign-on bonus paid over 26 payments, reflecting the organization’s commitment to attracting compassionate and capable team members.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of Federal and State long-term care regulations.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) with emphasis on comfort, dignity and safety.
• Supervise nursing assistants and medication aides during the shift per policy.
• Monitor residents’ status and notify physician of changes.
• Develop and review individualized care plans with the interdisciplinary team.
