Peak Rehabilitation Hospital, part of Lifepoint Health, is seeking a licensed practical nurse (LPN) for full-time Night shift 7p–7a with rotating weekends, on-call, and holidays. You will administer medications, assist RNs, and contribute to rehabilitation-focused patient care in a 52-bed inpatient setting.

Must hold an active LPN license, CPR/BLS, and graduate from an accredited LPN program. IV certification preferred or obtainable within 6 months.

• Must hold current/active LPN certification and be graduate of an accredited LPN program.
• CPR/BLS certification required
• IV certification strongly preferred or must be obtained within 6 months of hire
• Minimum six months of Medical/Surgical experience in an acute care setting preferred
• Strong clinical, assessment, and patient care skills
• Effective communication and patient education abilities
• Ability to work collaboratively in an interdisciplinary team environment

Responsabilidades

• Administers medications safely and accurately based on patient condition and age
• Follows proper medication administration procedures to reduce risk of errors
• Assists the RN with patient assessments and reassessments in accordance with policy
• Identifies and implements appropriate rehabilitation nursing interventions
• Completes timely and accurate Quality Improvement (QI) assessments under RN supervision
• Delivers patient care based on treatment response and updates care plans as needed
• Assess and manages patient pain using appropriate interventions
• Educates patients and families on treatment plans and recovery expectations
• Promotes patient safety and helps reduce the risk of medical errors
• Develops individualized patient and family education plans
