Job summary
HSHS Rehabilitation Hospital is hiring a Licensed Practical Nurse for part-time night shifts in a newly opened 40-bed inpatient rehabilitation facility. You will deliver direct and indirect patient care using the nursing process and collaborate with an interdisciplinary team to ensure optimal outcomes.

Qualifications
- Graduation from an accredited nursing program and state licensure as LPN.
- Excellent oral and written communication abilities.
- Experience collaborating with interdisciplinary care teams is preferred.
- ACLS certification preferred but not required.
Responsibilities
- Provide direct and indirect patient care using the nursing process (Assessment, Planning, Intervention, Evaluation).
- Develop and coordinate individualized nursing care plans with patients and families.
- Communicate patient condition changes to Physicians and Nursing leadership.
- Assist in discharge planning to support safe transitions.
