Quality Center for Rehabilitation and Healing is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our night-shift clinical team. The role requires providing high-quality nursing care in a rehabilitation and long-term care setting, with emphasis on resident-centered care and teamwork.

We look for candidates with an unrestricted LPN license and nursing program diploma, plus experience in skilled nursing or long-term care. CPR is preferred or willingness to obtain.

Qualifications

• Current, unrestricted LPN license in the state of employment.
• Graduate of an accredited practical nursing program.
• Previous skilled nursing, rehabilitation, or long-term care experience preferred.
• Current CPR certification or willingness to obtain it, as required.
• Strong clinical, communication, and organizational skills.
• Compassionate and resident-focused approach to care.
• Ability to work effectively in a fast-paced healthcare environment.
• Dependable, professional, and committed to teamwork.
• Availability to work the 6:00 p.m.-6:00 a.m. night shift.
• Ability to meet the physical requirements of the position.

Responsabilités

• Provide direct nursing care in accordance with residents' care plans.
• Administer medications and treatments as ordered by physicians.
• Monitor residents' conditions and promptly report changes to the appropriate clinical team members.
• Accurately document medications, treatments, observations, and resident responses.
• Assist with admissions, transfers, and discharges.
• Communicate with residents, families, physicians, and members of the interdisciplinary team.
• Supervise and provide guidance to Certified Nursing Assistants as assigned.
• Follow infection-control, safety, and emergency procedures.
• Maintain residents' dignity, privacy, comfort, and confidentiality.
• Ensure compliance with facility policies and applicable nursing regulations.
• Respond appropriately to resident needs and clinical emergencies.
• Participate in care planning, shift reports, and quality-improvement initiatives.
• Perform other nursing duties as assigned.
