Peak Rehabilitation Hospital in Apex, NC is seeking a Licensed Practical Nurse (LPN) for a full-time Days shift (7a–7p) with rotating weekends, on-call, and holidays. The role involves medication administration, patient assessments with RN support, and contributing to rehabilitation nursing plans in a 52-bed inpatient setting.

Applicants must hold an active LPN license and CPR/BLS. IV certification is preferred or to be obtained within 6 months.

Qualifications

• Must hold current/active LPN certification.
• CPR/BLS certification required; IV certification preferred or to be obtained within 6 months.

Responsibilities

• Administers medications safely and accurately based on patient condition and age.
• Follows proper medication administration procedures to reduce risk of errors.
• Assists the RN with patient assessments and reassessments per policy.
• Identifies and implements rehabilitation nursing interventions.
• Completes timely and accurate Quality Improvement assessments under RN supervision.
• Delivers patient care based on treatment response and updates care plans as needed.
• Assesses and manages patient pain and educates on treatment plans.
• Promotes patient safety and reduces the risk of medical errors.
• Develops individualized patient and family education plans.
