Job summary

Community North Rehabilitation Hospital in Indianapolis, IN is seeking a Licensed Practical Nurse (LPN) for a 36-hour weekly night shift (12-hour shifts 6:30 PM–7:00 AM). You will deliver direct patient care using the nursing process in a rehabilitation setting, under RN supervision.

Responsibilities include medication administration, patient assessments, care plan updates, pain management, and thorough documentation. Indiana LPN license, BSN/ADN/diploma, CPR/BCLS; ACLS preferred.

Qualifications

• Indiana LPN license required.
• Graduate from an accredited BSN/ADN or nursing diploma program.
• Current state licensure as Licensed Practical Nurse in Indiana.
• CPR/BCLS certification; ACLS preferred.

Responsibilities

• Demonstrates knowledge of medications and their correct administration based on patient age and clinical condition.
• Performs patient care considering needs specific to rehab inpatients and geriatric patients.
• Under RN supervision, assists with patient assessment and reassessments and initiates rehab nursing interventions.
• Under RN supervision, conducts timely and accurate QI assessments.
• Carries out the care plan as indicated and updates the interdisciplinary team as patient condition changes.
• Interacts professionally with patient and family in care planning.
• Assesses and addresses pain, educating patient and family on pain management.
• Performs other duties as assigned.
