Job summary

Villas of Holly Brook is seeking a Licensed Practical Nurse (LPN) for a full-time professional role in Peoria, IL. The position involves direct resident care, coordination with the Wellness Director, and support for staff within a memory care community.

Qualifications

• Licensed Practical Nurse (LPN) licensed to practice as required by State Regulations.
• Experience in direct patient care and assessments/observation of clinical conditions.
• CPR certified and knowledge of basic first aid.
• Must complete all state required training to maintain licensure.

Responsibilities

• Provide resident care and support to the Wellness Director and staff.
• Assist in supervising medication program and residents with self-administration.
• Assist with Activities of Daily Living as needed.
• Maintain positive relations with residents, families and physicians.
• Report significant changes to Executive Director/Wellness Director.
• Assist in resident assessments and service plans.
• Assist in training staff and participate in orientation.
