Qualifications

• Active LPN license in good standing.
• CPR and First Aid certification.
• Strong assessment and documentation skills.
• Experience in senior care or medication administration preferred.

Responsibilities

• Administer medications and treatments per physician orders.
• Follow the Six Rights of Medication Administration.
• Document in MAR/TAR accurately and in real time.
• Monitor symptoms and administer medications as ordered for symptom relief.
• Report refusals or adverse reactions promptly.
• Conduct SHUs and transcribe physician orders to support care plans.
• Observe and report physical, emotional, or cognitive changes.
• Maintain medication carts and complete narcotic counts; uphold compliance.
