Job summary

LiveWell Homecare Agency is seeking a Licensed Practical Nurse (LPN) Medication Nurse to safely administer medications, monitor residents, and document care in a senior living setting.

You will follow physician orders and Six Rights of Medication Administration, document in MAR/TAR in real-time, and support care plan updates. Candidates should have CPR/First Aid certification and experience in senior care or medication administration.

Qualifications

• CPR and First Aid certification.
• Strong assessment, observation, and documentation skills.
• Experience in senior care or medication administration preferred.

Responsibilities

• Administer medications and treatments following physician orders and the Six Rights of Medication Administration
• Accurately document in MAR/TAR in real-time
• Monitor residents for symptoms, adverse reactions, and changes in physical, emotional, or cognitive status
• Support care plan updates by transcribing physician orders and conducting Service & Health Updates (SHUs)
• Report any medication refusals, adverse reactions, or health concerns promptly
• Maintain medication carts, perform narcotic counts, and ensure regulatory compliance
