Job summary

LiveWell Homecare Agency is seeking a Licensed Practical Nurse (LPN) Medication Nurse to provide licensed nursing support in a senior living environment, focusing on safe medication administration and clinical documentation.

You will administer medications per physician orders, document in MAR/TAR in real time, monitor residents for changes, and maintain medication carts with strict regulatory compliance, SHUs, and safety.

Qualifications

• CPR and First Aid certifications required or preferred.
• Strong assessment and documentation skills.
• Experience in senior care or medication administration is preferred.

Responsibilities

• Administer medications and treatments according to physician orders, following the Six Rights of Medication Administration.
• Accurately document in MAR/TAR in real-time and support care plan updates through transcribing physician orders.
• Monitor residents for symptom relief, adverse reactions, and physical, emotional, or cognitive changes, reporting any concerns promptly.
• Maintain medication carts, conduct narcotic counts, and ensure compliance with regulatory standards.
• Conduct SHUs and support resident safety through clinical precision.
