Job summary

Preferred Home Health Care & Nursing Services is seeking an LPN to provide skilled nursing care in collaboration with RNs and UNLs based on patient acuity and load. You will perform medication administration, IV therapy, trach care, wound care, NG tube care, ostomy management, and patient education, while charting assessments and contributing to care plans.

The role requires an active LPN license in Illinois, BLS within 30 days, and a practical nursing diploma.

Qualifications

• LPN licensed by the IDFPR.
• BLS certification within 30 days of hire from the AHA.
• College diploma in Practical Nursing.
• Experience in medication passing, IV therapy, wound care, and patient assessments.
• Strong communication skills to coordinate with healthcare team and reinforce patient education.

Responsibilities

• Administer medications and IV fluids (excluding IV push meds).
• Chart assessments and update care plans as directed by RNs.
• Perform dressing, trach, NG tube, and ostomy care.
• Collaborate with physicians and APPs for care coordination.
• Provide patient education and discharge instructions.

Qualifications & Skills

• Licensed Practical Nurse (LPN) licensed by the Illinois Department of Financial and Professional Regulation (IDFPR)
• Basic Life Support (BLS) certification within 30 days of hire from the American Heart Association (AHA)
• College diploma in Practical Nursing
• Experience in medication passing, IV therapy, wound care, and patient assessments
