Job summary

Personal Touch Home Health Care Services Inc. seeks an LPN to provide clinical support in a patient‑centered setting, collaborating with RNs and UNLs to deliver care, especially when patient acuity is high or workloads are heavy.

Qualifications

Job description

Role Overview:

This position involves providing clinical support as a Licensed Practical Nurse (LPN) in a healthcare setting. The LPN collaborates with Registered Nurses (RNs) and Unlicensed Nurse Leaders (UNLs) to deliver patient care, especially when patient acuity is high or patient loads are heavy. Responsibilities include administering oral medications, hanging IV piggybacks and fluids (excluding IV push meds), performing dressing and trach care, inserting and caring for NG tubes, managing ostomies, and assisting with discharge education.

Key Responsibilities

• Pass medications and manage IV fluids as directed.


• Chart assessments and care plans in coordination with RNs.


• Communicate with physicians and advanced practice providers to coordinate care and receive orders.


• Perform wound dressing changes, trach care, NG tube insertions, and ostomy care.


• Reinforce patient education and discharge instructions.

Qualifications & Skills

• Licensed Practical Nurse (LPN) certification from the Illinois Department of Financial and Professional Regulation (IDFPR).


• Basic Life Support (BLS) certification within 30 days.


• Educational requirement: College diploma as an LPN.


• Strong communication skills and ability to collaborate with healthcare team members.
