Transitions Healthcare Gettysburg is currently hiring an LPN Assessment Coordinator. The primary purpose of this position is to work with the MDS Coordinator to plan and implement the MDS completion in accordance with facility procedures as well as State and Federal regulations.

LPN MDS Coordinator Responsibilities:

1. Gathers necessary data for completion of the MDS.
2. Assists in identifying significant changes in residents' condition.
3. Monitors resident care plans for accuracy and completion.
4. Attends the Medicare meeting.
5. Assists in educating nursing staff and the interdisciplinary care team about the MDS process.
6. Performs data entry of MDS information.
7. Reviews messages from contracted pharmacy regarding medications needing prior authorization.

Minimum Qualifications:

• Current licensure as either a practical nurse or a registered nurse in Pennsylvania or Multistate License.
• CPR/AED certification.
• Minimum of one (1) year experience with MDS 3.0.
• Proficient in MS Office Suite and Outlook.
