Responsibilities

  • Complete MDS, CAA’s, and care plans within regulated timeframes
  • Coordinate scheduling of the RAI process with the interdisciplinary team
  • Assess residents through physical exams, interviews, and chart reviews
  • Discuss resident care needs with caregivers, including physicians, nursing, social services, therapy, dietary, and activity staff
  • Review information from hospitals, consults, and outside agencies to inform assessments and care planning
  • Coordinate, identify, and initiate significant change MDSs
  • Conduct PPS meetings, maintain MDS assessments per Medicare schedule, and monitor Medicare days and RUGs utilization in the absence of the Care Management Coordinator
  • Stay current with AANAC requirements

Qualifications

  • RN or LPN licensure
  • AANC certification is a plus; RAC-CT preferred
  • Knowledge of the RAI process and PPS principles
  • Experience as an MDS Nurse
