Job summary

Distinctive Living in Maumee, OH is seeking a full-time LPN/LVN to assess resident health status and develop care plans under the Director, Health and Wellness. The role requires collaboration with nursing staff, medication aides, and administration to ensure regulatory compliance and high-quality care.

The successful candidate will have a nursing license, 1 year of relevant experience, supervisory exposure, and proficiency with Word/Excel.

Qualifications

• High School Diploma or equivalent required; LPN/LVN licensure required.
• Minimum of 1 year nursing or caregiver experience in assisted living, home health, or memory care.
• At least 6 months supervisory experience.
• Experience with Microsoft Word and Excel preferred.
• Must be able to read physician orders and interpret Medication Administration Records.

Responsibilities

• Follows resident service plans and documents changes in condition.
• Communicates resident needs to the Director of Health and Wellness as needed.
• Provides hands-on care and supports the resident's care plan in line with state regulations.
• Provides training to care staff and supports continuing education requirements.
• Transcribes physicians' orders and reviews new orders.
• Maintains required certifications and completes in-service trainings.
• Ensures compliance with HIPAA, OHSA, and Blood Borne Pathogen guidelines.
• Promotes teamwork and safety in the community.
