Job summary

Providence is seeking an LPN/LVN to participate, coordinate, and provide direct nursing care for participants. You will work closely with physicians, RNs, transportation, and home care to ensure satisfaction and retention in the community.

Responsibilities include vital signs, wound care, medication management, treatments, and patient education. A 5,000 USD hiring bonus is available for eligible external candidates who meet qualifications and payment conditions.

Qualifications

• Must be a Washington Licensed Practical Nurse at time of hire.
• CPR and First Aid Certification (American Heart Association) at time of hire.
• At least 1 year of long-term care or geriatric experience.
• Experience working with frail and elderly.

Responsibilities

• Provide direct nursing care and coordinate with the physician, RN, transportation and home care.
• Assist with vital signs, wound care, medication administration, treatments, and venipuncture.
• Assist with catheter care, foot care, and setting up for physician visits.
• Support Providence PACE in keeping older adults healthy and living in the community.

Job Description

The LPN / LVN participates, coordinates and provides direct nursing care in a manner which ensures participant's satisfaction and retention. Coordinating with the physician, RN, transportation and home care. Assists physicians and nursing staff with a variety of tasks, including but not limited to vital signs, wound care, medication administration/management, treatments, patient teaching, venipuncture, catheter care foot care, and participant set up for physician visit. Providence PACE is a Program of All-Inclusive Care for the Elderly that strives to keep older adults as healthy as possible living in the community through clinics, home visits and more. Required Qualifications

• Education to meet certification, license or registration requirement.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
