Job summary

Heart to Heart Hospice is seeking an LPN/LVN On-Call to support patient care in a hospice setting. The role involves supplementing nursing care as directed, performing home visits, and assisting in implementing the Plan of Care under the On Call RN/Triage RN.

The position requires graduation from an accredited nursing program, an active LVN/LPN license (license preferred), two years of relevant experience, a valid driver's license, and reliable transportation with insurance.

Qualifications

• Graduate of an accredited school of vocational nursing.
• Current State License as an LVN/LPN preferred.
• Two years of LVN/LPN experience in a clinical care setting, hospice or home health preferred.
• Current State Driver's License.
• Must have reliable transportation and provide ongoing valid and current auto liability insurance.

Responsibilities

• Under the direction of the On Call RN/Triage RN, supplements the nursing care needs of the patient as provided by the RN and as specified in the Plan of Care (POC).
• Provide home visits as directed by the On Call RN/Triage RN to observe and report findings to the appropriate RN.
• Assist the RN in caring out the POC.
• Assist the patient in learning appropriate self-care techniques.
• Provide services in accordance with organizational policies and procedures and regulations that define the scope of services.
• Assist the RN and/or physician in performing specialized duties related to end-of-life care.
• Timely submission of all documentation.
• Performance of other duties as required.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
