Job summary

Heart to Heart Hospice is seeking a dedicated LVN/LPN On Call for night shifts (Friday and Saturday) in the San Diego area. Under the On Call RN/Triage RN, you will supplement patient care and help implement the Plan of Care in a home hospice setting.

You will perform home visits, assist with end-of-life care, document timely, and ensure compliance with laws and regulations. A valid LVN/LPN license, a accredited nursing program, and a reliable vehicle are required; hospice/home health experience

Qualifications

• Current Vocational Nurse or Practical Nurse license.
• Graduate of an accredited school of vocational nursing.
• Valid driver's license and reliable transportation with ongoing auto liability insurance.
• Two years of LVN/LPN experience in a clinical care setting; hospice/home health preferred.

Responsibilities

• Support nursing care needs under the direction of the On Call RN/Triage RN and the Plan of Care.
• Provide home visits as directed to observe and report findings to the RN.
• Assist the RN in carrying out the Plan of Care.
• Assist the patient in learning appropriate self-care techniques.
• Provide services in accordance with organizational policies and procedures and regulations.
• Assist the RN/physician with end-of-life care duties.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state and federal laws and Medicare regulations.

What You Must Have

• Current Vocational Nurse State Licenseor Practical Nurse License

• Two years of experience as a Licensed Vocational Nurse or Licensed Practical Nurse in a clinical care setting preferred, hospice or home health preferred

What You Will Do

• Under the direction of the On Call RN/Triage RN, supplements the nursing care needs of the patient as provided by the RN and as specified in the Plan of Care (POC)
• Provide home visits as directed by the On Call RN/Triage RN to observe and report findings to the appropriate RN.
• Assist the RN in caring out the POC
• Assist the patient in learning appropriate self-care techniques
• Provide services in accordance with organizational policies and procedures and regulations that define the scope of services
• Assist the RN and/or physician in performing specialized duties related to end-of-life care
• Timely submission of all documentation
• Performance of other duties as required
