Job summary

Heart to Heart Hospice seeks a Registered Nurse On Call to deliver compassionate care in patient homes. You will assess, treat, and support patients during visits and coordinate with physicians to update care plans.

Applicants must hold a State RN license, a valid driver’s license, reliable transportation, and have 2 years of RN experience preferred in clinical settings. This full-time role includes strong benefits and mileage reimbursement.

Qualifications

• Graduate of an accredited nursing program (Diploma/Associate/Bachelor).
• Current State Registered Nurse license.
• Valid driver's license with reliable transportation and ongoing auto liability insurance.
• Two years of RN experience preferred in a healthcare/clinical setting.

Responsibilities

• Provide home visits for patient assessment, treatment, and support, including pronouncement of death, crisis management and admission.
• Develop and revise nursing care plans with goals in collaboration with physician and IDG.
• Use excellent customer care skills with internal and external customers, including phone etiquette.
• Prepare clinical and progress notes showing progress toward goals.
• Inform physician and staff of changes in patient needs and intervention outcomes.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with laws, Medicare regulations, and personnel policies.

What You Will Do:

• Provide home visits as needed for patient assessment, treatment, and support. Visits may include pronouncement of patient death, crisis management and admission
• Plans for comprehensive service to patients and their families; develops and/or revise nursing care plans, including goals in conjunction with the patient’s physician and IDG

• Inform physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care
