Job summary

Heart to Heart Hospice is seeking a Licensed Vocational Nurse/Licensed Practical Nurse to join our care team in Texas. You will work under the RN, support the plan of care, and help patients and families with essential self-care techniques.

This full-time role requires an active LVN/LPN license, graduation from an accredited program, a valid driver's license, and ideally hospice experience. Benefits include medical/dental/vision, PTO, holidays, 401k matching, tuition reimbursement, and a company

• Graduate of an accredited school of vocational nursing.
• Two years of clinical nursing experience; hospice or home health preferred.

Responsibilities

• Assist the RN in carrying out the plan of care.
• Under the RN direction, supplement nursing care needs as specified in the POC.
• Assist the patient in learning appropriate self-care techniques.
• Complete clinical and progress notes, documenting outcomes of interventions.
• Assess patient/family response to care and communicate with the team.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state and federal laws, Medicare regulations and policies.

What You Will Do:
