Job summary

Qualifications

• Graduation from an accredited LPN/LVN certification program.
• Licensed by the State of Texas as an LVN.
• Minimum of two years’ experience as an LPN/LVN.
• Ability to communicate in English, both verbally and in writing.
• Ability to document in electronic health records and utilize Microsoft Office applications.

Responsibilities

• Serve as liaison between patient, family, PCP, and multidisciplinary care team.
• Coordinate care plans and transitions across care settings.
• Assess needs and coordinate resources to promote optimal health and outcomes.
• Communicate treatment plans and patient status to all care givers during transitions.
