Qualifications

• Active and in good standing state LVN or LPN license.
• NCLEX-PN completed.
• Knowledge of medical terminology and clinical documentation standards.
• Experience with phlebotomy, vaccine administration, and point-of-care testing.
• Excellent communication and organizational skills.
• Experience in clinical settings with diverse cultures.
• Ability to handle sensitive information ethically.
• Strong problem-solving and critical thinking.
• EMR proficiency; Epic experience preferred.

Responsibilities

• Administer oral, injectable, IV, and topical meds.
• Evaluate complex patient conditions and document responses.
• Monitor vital signs and record abnormalities.
• Perform routine health assessments.
• Coordinate access to care and connect to community resources.
• Conduct medication reviews for interactions.
• Develop mental health assessments to screen depression.
• Document medical history in EMR.
• Communicate with patients, families and healthcare teams.
• Maintain organized, clean clinic.
