Job summary

Talented Medical Solutions is seeking a Licensed Practical Nurse to provide nursing care in a long-term care setting under supervision. The role includes administering medications, monitoring residents, and ensuring accurate documentation.

You will assess residents, supervise CNAs, and communicate changes in condition to the care team, RN, physician and family as appropriate while upholding infection control standards and regulatory requirements.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Current LPN/LVN license in the state of practice.

Responsibilities

• Administer medications and treatments per orders and facility policy.
• Perform resident assessments and monitor changes in physical, cognitive or behavioral status.
• Supervise and delegate to CNAs as appropriate.
• Complete accurate documentation in the resident record.
• Communicate changes in condition to the RN, physician and family as appropriate.
• Follow infection control and regulatory requirements for long-term care.

Job Summary

Delivers nursing care to residents in a long-term care setting under the direction of a Registered Nurse or physician. Administers medications and treatments, monitors resident status and supervises Certified Nursing Assistants. Maintains accurate clinical documentation and communicates changes in condition to the care team.

Education & Experience

• Graduate of an accredited practical or vocational nursing program: Required
• Long-term care or skilled nursing experience: Preferred

Licenses & Certifications

• Licensed Practical Nurse or Licensed Vocational Nurse licensure in the state of practice: Required
• Basic Life Support (BLS) certification: Required
