Job summary

Citadel At Saint Benedict is seeking a Licensed Practical Nurse (LPN) to join their nursing team in a caring long-term care setting. The LPN will assume responsibility for a group of residents during a shift, providing licensed care as ordered by physicians and following federal, state, and local standards.

The role includes taking vital signs, administering medications, documenting care, and coordinating with the IDT for care plan updates.

Qualifications

• Current CPR Certification required.
• Licensed to practice in Illinois as an LPN.
• 1 year of experience preferred.
• Long-Term care experience preferred.

Responsibilities

• Provides licensed care to residents as ordered by physician and in accordance with standards and regulations.
• Receives nursing reports at shift start and end.
• Maintains clean and sanitary work areas and treatment areas.
• Directs and supervises nursing assistant services for the unit to ensure care compliance.
• Performs resident assessments/observations and reviews care plans.
• Takes vital signs and administers medications as ordered.
• Documents care and resident responses in charts and MAR.
• Completes admission, transfer, and discharge forms.
• Ensures safety and addresses hazards in the resident environment.
• Reports on shift activities and recommendations for care plan changes.
