Job summary

Osceola Regional Health Center in Sibley, IA seeks a Licensed Practical Nurse - LTC to provide direct nursing care for residents within the long-term care setting. You will support the nursing process, develop plans of care, and collaborate with an interdisciplinary team to deliver high-quality resident care.

Responsibilities include assessments under RN supervision, care coordination, and supervising nursing staff.

Qualifications

• Graduate of an accredited school of nursing.
• Two years nursing and/or long-term care experience preferred.
• LPN license in Iowa required (active).
• CPR certification within 3 months of hire.
• Iowa abuse mandatory reporter training within 6 months of hire.

Responsibilities

• Assist RN in documenting and executing the nursing process for resident care.
• Perform initial and follow-up assessments under RN direction.
• Contribute to developing and updating the resident care plan.
• Coordinate services across the care continuum and educate residents/families.
• Supervise unlicensed personnel and ensure resident rights are upheld.
• Participate in committees and ongoing professional development.
