Job summary

As an LPN at Oak Hill, you will provide hands-on nursing support, coordinate healthcare services, monitor medical needs, and work collaboratively with interdisciplinary teams to ensure high-quality, person-centered care. Weekly Hours

Oak Hill Substitutes must work a minimum of 15 hours/month (average)

Weekly Schedule:

Substitutes work on a per-diem (as needed) basis.

Range of competitive hourly rate, dependent upon experience

What You'll Do

As a Licensed Practical Nurse, you'll support the health and well-being of individuals served by Oak Hill through a combination of clinical care, health monitoring, documentation, and direct support by:

Providing Clinical and Nursing Care
  • Monitor, assess, and respond to the health needs of individuals served.
  • Follow physician orders and nursing directives to ensure appropriate care and treatment.
  • Administer medications, injections, treatments, and other prescribed interventions.
  • Monitor vital signs and health conditions, documenting observations accurately and timely.
  • Support specialized medical needs, including G-Tube care and other physician-directed health interventions.
  • Respond appropriately to medical emergencies and coordinate emergency care when needed.
  • Schedule and coordinate medical appointments and follow-up care.
  • Communicate with physicians, pharmacies, hospitals, guardians, and interdisciplinary team members.
  • Maintain medication records, nursing notes, medical consultation forms, and other required documentation.
  • Monitor medication inventories, order supplies, and ensure individuals have access to prescribed medications and treatments.
Promoting Health, Safety, and Wellness
  • Assess health concerns and make recommendations regarding participation in daily activities, programs, and community outings.
  • Observe and report changes in physical, behavioral, or mental status.
  • Support preventive healthcare initiatives and wellness education.
  • Implement health-related policies, procedures, and individualized care plans.
Supporting Daily Living and Independence
  • Provide guidance and assistance with personal care activities, including bathing, dressing, grooming, and hygiene when needed.
  • Encourage independence and skill development while promoting dignity and self-esteem.
  • Assist individuals with meal preparation, feeding support, and adherence to dietary guidelines.
  • Support participation in recreational, vocational, and community activities.
Maintaining Documentation and Regulatory Compliance
  • Complete nursing notes, incident reports, medical reviews, and other required records.
  • Participate in interdisciplinary team meetings and provide health-related updates.
  • Ensure compliance with all Oak Hill, DDS, DPH, HIPAA, and regulatory requirements.
  • Maintain confidentiality and accurate health records at all times.
Supporting a Safe and Inclusive Environment
  • Apply First Aid, CPR, and emergency response procedures as necessary.
  • Utilize approved lifting and transfer techniques, including Hoyer lifts and adaptive equipment.
  • Assist with behavior support plans and other individualized programming as needed.
  • Safely transport individuals to medical appointments, community activities, and other destinations using agency vehicles.
Collaborating as a Team Member
  • Communicate effectively with colleagues, supervisors, nursing staff, families, and healthcare providers.
  • Participate in staff meetings, required trainings, certifications, and professional development activities.
  • Assist with orienting and mentoring new employees as needed.
  • Contribute to a positive, mission-driven team environment focused on improving live s.

Required Qualifications
  • Current Connecticut Licensed Practical Nurse (LPN) license in good standing.
  • Graduate of an accredited Practical Nursing program.

  • Experience supporting individuals with intellectual and developmental disabilities.
  • Experience in residential, community-based, long-term care, behavioral health, or human services settings.
  • Experience with medication administration, G-Tube care, and health record documentation.
  • Familiarity with DDS regulations and person-centered support practices.
