Responsibilities

  • Support and educate residential staff on healthcare plans, medication protocols, and individual health needs.
  • Respond appropriately to medical concerns and emergencies.

Oak Hill is seeking a highly motivated and dynamic Licensed Practical Nurse (LPN) to assist in maintaining and improving the health and wellbeing of individuals by monitoring health status and providing appropriate treatments to individuals with Developmental and Intellectual Disabilities to enable their highest quality of life in terms of their physical, emotional, and mental health while adhering to Oak Hill’s policies and procedures. Schedule: Mon-Fri 7am-3pm

We are looking for a compassionate and skilled Licensed Practical Nurse (LPN) to join our team and provide essential nursing support to individuals with disabilities living in a residential group home setting.

$4,000 Sign-On Bonus! Sign-on bonus subject to applicable eligibility requirements and Oak Hill's bonus agreement.

What You'll Do

As an LPN, you will play a critical role in supporting the health, safety, and well-being of the individuals we serve. Working collaboratively with Direct Support Professionals, program leadership, physicians, families, and interdisciplinary team members, you will:

  • Provide direct nursing care and ongoing health monitoring for individuals living in a group home setting.
  • Administer medications and treatments in accordance with physician orders and applicable regulations.
  • Monitor, assess, and document changes in individuals' health status.
