Job summary

Oak Hill is seeking a Licensed Practical Nurse (LPN) for a full-time overnight role in a residential group home in Wethersfield, CT. The LPN will monitor health, administer medications, and coordinate care with an interdisciplinary team to support individuals with developmental disabilities.

Ideal candidates hold a CT LPN license, have strong assessment and documentation skills, and can work independently within a multidisciplinary team. A $4,000 sign-on bonus is available to eligible applicants.

Qualifications

• Current Connecticut Licensed Practical Nurse (LPN) license in good standing.
• Experience with individuals with intellectual and developmental disabilities is preferred but not required.
• Strong clinical assessment, communication, and documentation skills.
• Valid driver's license and ability to meet Oak Hill driving requirements.

Responsibilities

• Provide direct nursing care and ongoing health monitoring in a group home.

We are looking for a compassionate and skilled Licensed Practical Nurse (LPN) to join our team and provide essential nursing support to individuals with disabilities living in a residential group home setting.

• Support and educate residential staff on healthcare plans, medication protocols, and individual health needs.
• Respond appropriately to medical concerns and emergencies.
• Promote independence, dignity, choice, and the highest possible quality of life for every individual served.
• Collaborate with an interdisciplinary team to ensure coordinated, person-centered care.
