Great Lakes Management Company is seeking an LPN to join Global Pointe Senior Living in Golden Valley, MN. The role supports senior residents with daily care and clinical needs, ensuring safety and quality of life under state regulations.

Qualifications

• Current MN LPN license.
• Minimum one year in home health, assisted living, or long-term care.
• Good organizational skills with minimal supervision.

Responsibilities

• Assures residents meet admission criteria and recommends placement when needed.
• Ensures residents have up-to-date care plans per state regulations.
• Monitors and maintains resident charts and progress notes.
• Communicates changes in residents' needs to physicians, staff and family.
• Coordinates resident services with other providers.
• Oversees medications and renewals and follows up with physicians.
• Ensures proper medication administration procedures and addresses corrections with staff.
• Documents in resident logs for staff awareness.
• Provides alternating supervisory visits with the RN.
• Works as a team with the residents' care team.
• Supervises medication practices and equipment used in the building.
• Ensures staff follow all care procedures.
• Shares resident and family concerns with pertinent staff.
• Collaborates with management on emergency procedures.
• Assists in managing the clinical schedule with leadership.
• On-call rotation availability.

LPN Responsibilities:

• Assures residents maintain admission criteria, monitor, and recommend alternative placement if necessary.
• Assures residents have up to date care plans per state regulations
• Monitors and maintains resident charts and clinical progress notes.
• Communicates to physicians, staff and family changes in resident's needs or conditions.
• Facilitates coordination of resident services with other service providers.
• Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
• Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
• Communicates in resident's log any information that staff need to be aware of.
• Provides alternating supervisory visits in coordination with the RN.
• Works as a team player with all members of the residents' care team.
• Supervises medication practices and medical equipment being used in the building.
• Assures that staff clearly understand and follow all resident care procedures.
• Shares resident and family concerns with pertinent staff if appropriate.
• Works with manager to assure emergency procedures are in place
• Works with clinical leadership to manage the clinical schedule.
• Able to be in an on call rotation.

LPN Qualifications:

• Maintains a current state license as a n LPN (Licensed Practical Nurse) per state regulations
• A minimum of one year's experience in home health, assisted living or long-term care is preferred
• Good organizational skills with the ability to self-start and follow through with initiatives with a minimal amount of supervision
• Exhibits a caring heart with a passion to care for the elderly
