Job summary

Great Lakes Management in Minnetonka, MN is seeking an LPN to join our assisted living team. You will monitor residents under state regulations, maintain care plans, and coordinate with physicians and the care team to ensure safe, compassionate services.

LPNResponsibilities:

• Assures residentsmaintainadmission criteria,monitor, and recommend alternative placement if necessary.
• Assures residents have up to date care plans per state regulations
• Monitors andmaintainsresident charts and clinical progress notes.
• Communicates to physicians, staff and family changes in resident’s needs or conditions.
• Facilitates coordination of resident services with other service providers.
• Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
• Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
• Communicates in resident’s log any information that staff need to be aware of.
• Provides alternating supervisory visits in coordination with theRN.
• Works as a team player with all members of the residents’ care team.
• Supervises medication practices and medical equipment being used in the building.
• Assures that staff clearlyunderstandand follow all resident care procedures.
• Shares resident and family concerns with pertinent staff ifappropriate.
• Works with manager to assure emergency procedures are in place

LPNQualifications:

• Maintains a current state license as an LPN(Licensed Practical Nurse)per state regulations
• A minimum of oneyear'sexperience in home health, assisted living orlong-termcare is preferred
• Good organizational skills with the ability to self-start and follow through with initiatives with a minimal amount of supervision
• Exhibitsa caring heart with a passion to care for the elderly
