Job summary

Great Lakes Management is seeking an LPN to join our team in Colorado. The role focuses on advancing resident care in assisted living communities, with responsibilities including care plans, medication oversight, and coordination with physicians and families.

Pay range is $32-$38 per hour, with a schedule featuring Sundays, Tuesdays, and Wednesdays from 8am to 8pm. Benefits include 401(k) match, health coverage, paid vacation and sick time, floating holidays, and free meals per shift.

Qualifications

• Current LPN license in the state of Colorado.
• Minimum of one year experience in home health, assisted living or long-term care preferred.
• Good organizational skills with ability to self-start and follow through with minimal supervision.
• Caring heart with a passion to care for the elderly.

Responsibilities

• Assures residents maintain admission criteria, monitor, and recommend alternative placement if necessary.
• Assures residents have up-to-date care plans per state regulations.
• Monitors and maintains resident charts and clinical progress notes.
• Communicates changes in residents' needs or conditions to physicians, staff and families.
• Facilitates coordination of resident services with other service providers.
• Oversees medications and renewals and follow-up with physicians as needed.
• Ensures medication administration procedures are followed and discusses corrections with staff.
• Communicates in the resident’s log any information staff need to be aware of.
• Provides alternating supervisory visits in coordination with the RN.
• Works as a team player with all care team members.
• Supervises medication practices and medical equipment in use.
• Ensures staff understand and follow all care procedures.
• Shares resident and family concerns with pertinent staff.
• Works with manager to ensure emergency procedures are in place.

LPN Qualifications:

• Maintains a current state license as an LPN (Licensed Practical Nurse) per state regulations
• A minimum of one year's experience in home health, assisted living or long-term care is preferred
