Job summary

Great Lakes Management is seeking an LPN to join our senior living team in Golden Valley, MN. This full-time role requires MN LPN certification and at least one year in home health, assisted living, or long-term care.

You will oversee medications, care plans, and charts while coordinating with physicians and family. The position offers competitive pay ($31.00–$33.00 hourly), 401(k) match, health benefits, paid vacation and sick time, floating holidays, and a free meal per shift.

Qualifications

• State license as an LPN in MN.
• Minimum 1 year experience in home health, assisted living or long-term care preferred.
• Good organizational skills with ability to self-start and follow through with minimal supervision.
• Caring heart with a passion to care for the elderly.

Responsibilities

• Assures residents meet admission criteria, monitor, and recommend alternative placement if necessary.
• Ensures residents have up-to-date care plans per state regulations.
• Monitors and maintains resident charts and progress notes.
• Communicates changes in resident needs or conditions to physicians, staff and family.
• Facilitates coordination of resident services with other providers.
• Oversees medications and renewals and follows up with physicians as needed.
• Ensures medication administration procedures are followed and addresses needed corrections with staff.
• Records information in resident logs for staff awareness.
• Provides alternating supervisory visits in coordination with the RN.
• Collaborates with care team and manager to ensure emergency procedures are in place.
• Works with clinical leadership to manage the clinical schedule.
• May participate in on-call rotation.

LPN R esponsibilities:

• Assures residents maintain admission criteria, monitor , and recommend alternative placement if necessary.
• Assures residents have up to date care plans per state regulations
• Monitors and maintains resident charts and clinical progress notes.
• Communicates to physicians, staff and family changes in resident’s needs or conditions.
• Facilitates coordination of resident services with other service providers.
• Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
• Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
• Communicates in resident’s log any information that staff need to be aware of.
• Provides alternating supervisory visits in coordination with the RN .
• Works as a team player with all members of the residents’ care team.
• Supervises medication practices and medical equipment being used in the building.
• Assures that staff clearly understand and follow all resident care procedures.
• Shares resident and family concerns with pertinent staff if appropriate .
• Works with manager to assure emergency procedures are in place
• Works with clinical leadership to manage the clinical schedule.
• Able to be in an on call rotation.

LPN Qualifications :

• Maintains a current state license as a n LPN ( Licensed Practical Nurse ) per state regulations
• A minimum of one year's experience in home health, assisted living or long-term care is preferred
