Job summary

Westchester Village is seeking an LPN Charge Nurse (PRN) to join our dedicated team. You will plan, organize, and assist in providing direct and indirect resident care following established policies and nursing standards.

The role includes supervising staff, ensuring safety, and collaborating with the DON and physicians to optimize resident well-being in a caring community setting.

Qualifications

• Graduate from an accredited school and licensed in the applicable state.
• 1–2 years of nursing experience as a Charge Nurse in Long Term Care or Skilled Nursing Facility preferred.
• CPR certification required.

Responsibilities

• Conduct daily rounds to assess resident condition and comfort.
• Assist DON/RN in developing and evaluating plans of care.
• Oversee direct care staff and ensure tasks are completed as assigned.
• Communicate with physicians and ensure hand-off between shifts.

The LPN Charge Nurse plans, organizes, and assists in providing direct and indirect resident care following established policies and procedures, standards of nursing practice and principles of nursing care.
