The LPN Nurse Navigator will guide incoming calls and facilitating entrance of new patients into the program. Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients. The Nurse Navigator, functioning in the spirit of teamwork, is accountable for the delivery of care utilizing skills learned in both the clinic and didactic arena. Assists other team members on the unit. The nurse navigator role provides clinical support to the provider and patients to assist in the diagnosis and treatment of conditions.
This role is full time, days, 40 hours weekly. At least two (2) years of LPN work experience is required.
