Phoenix Home Health in Warrensburg, MO is seeking a dedicated LPN to serve as the vital link between patients and providers, assessing needs and tailoring plans based on physician orders. You will collaborate with nurses, PT, OT, and ST to ensure comprehensive care.

This full-time role includes on-site charting with Roger's AI-powered support, a competitive pay range of $50,000–$80,000, and a benefits package. Schedule is Monday–Friday 8am–5pm with weekend rotation every 4–6 weeks.

Qualifications

• LPN license valid in Missouri or Compact state.
• Strong organizational and self-management skills.
• Strong and compassionate customer service skills.
• Valid driver's license.

Responsibilities

• Assists in review and revision of the plan of care.
• Provides nursing care in accordance with physician's plan of treatment and client plan of care.
• Observes, records and reports reactions to treatment and changes in client condition.
• Provides ongoing instruction of client or family in the plan of treatment.
• Participates in education programs, quality activities, nurse meetings and other staff meetings.

As a valued LPN at Phoenix Home Health, you will serve as the vital link between our patients and their medical providers. We are looking for someone with exceptional skills in physical assessment and communication, enabling you to execute tailored care plans based on physician orders while incorporating insights from initial assessments. You will collaborate closely with fellow nurses, Physical Therapists, Speech Therapists, and Occupational Therapists. Requirements

• Missouri or Compact Licensed Practical Nursing License

• Observes, records and report's reaction to treatment and any changes in client's condition to appropriate personnel and/or physician.

• Participates in education programs, quality assessment activities, nurse's meetings and other staff meetings as required.

Monday through Friday 8am to 5pm with every 4 to 6 weeks, weekend rotation
