Job summary

Phoenix Home Care and Hospice in Branson, MO seeks a dedicated LPN to join our home health team. You will provide nursing care according to physician orders and the client’s care plan, conducting tailored assessments and coordinating closely with nurses and therapists.

This full-time position offers competitive pay ($50,000–$80,000) and benefits, including medical plans, 401K, PTO, and paid mileage, with on-site charting and weekday hours. A MO/LPN license is required.

Qualifications

• Missouri or Compact RN/LPN license required.
• Strong organizational and self-management skills.
• Strong and compassionate customer service skills.
• Valid driver's license is required.

Responsibilities

• Review and revise the plan of care as needed.
• Provide nursing care per physician's plan of treatment and the client's plan of care.
• Observe, document, and report responses to treatments and changes in condition to the team/physician.
• Educate client or family on the plan of treatment.
• Participate in education programs, quality activities, and staff meetings.
• Work Mon-Fri, 8am–5pm with weekend rotation every 4–6 weeks.

You will collaborate closely with fellow nurses, Physical Therapists, Speech Therapists, and Occupational Therapists. Responsibilities

• Assists in review and revision of the plan of care
• Provides nursing care in accordance with physician's plan of treatment, individual plan of care, as authorized by client's payer source
• Observes, records and report's reaction to treatment and any changes in client's condition to appropriate personnel and/or physician
• Provides ongoing instruction of client or family in the plan of treatment
• Participates in education programs, quality assessment activities, nurse's meetings and other staff meetings as required
• Monday through Friday 8am to 5pm with every 4 to 6 weeks, weekend rotation
