Job summary

Phoenix Home Health in Lebanon, MO is seeking an LPN to join our home health team. You will serve as the vital link between patients and medical providers, crafting tailored care plans based on physician orders and initial assessments.

You will work with a multidisciplinary team and complete charting on-site with Roger's AI-powered support. We offer comprehensive benefits, a supportive culture, and opportunities for growth.

Qualifications

• Missouri or Compact Registered Nursing or Licensed Practical Nursing license is required.
• Strong organizational and self-management skills are essential.
• Excellent customer service skills and compassion are required.
• Valid driver’s license is required.

Responsibilities

• Review and revise the plan of care in collaboration with the physician.
• Provide nursing care per the physician's plan of treatment and the individual care plan.
• Observe and document responses to treatment and changes in condition.
• Educate patients and families about the plan of treatment.
• Participate in staff meetings, quality assessment activities, and ongoing education.
• Schedule is Monday–Friday, 8am–5pm with occasional weekend rotation.

We are looking for someone with exceptional skills in physical assessment and communication, enabling you to execute tailored care plans based on physician orders while incorporating insights from initial assessments. You will collaborate closely with fellow nurses, Physical Therapists, Speech Therapists, and Occupational Therapists. Responsibilities

• Assists in review and revision of the plan of care
• Provides nursing care in accordance with physician's plan of treatment, individual plan of care, as authorized by client's payer source
• Observes, records and report's reaction to treatment and any changes in client's condition to appropriate personnel and/or physician
• Provides ongoing instruction of client or family in the plan of treatment
• Participates in education programs, quality assessment activities, nurse's meetings and other staff meetings as required
• Monday through Friday 8am to 5pm with every 4 to 6 weeks, weekend rotation
