Job summary

Phoenix Home Care and Hospice is seeking a dedicated LPN to join our team in Missouri. You will provide nursing care per physician orders, develop tailored care plans, and coordinate with a multidisciplinary team to support patient health.

The role features a Monday–Friday 8am–5pm schedule with occasional weekend rotation and on-site charting supported by our AI-powered tools. A compassionate, patient-focused approach is essential to deliver New Beginnings through skilled home health care.

Qualifications

• Missouri or Compact LPN license.
• Strong organizational and self-management skills.
• Strong and compassionate customer service skills.
• Valid driver's license.

Responsibilities

• Assists in review and revision of the plan of care.
• Provides nursing care in accordance with physician's plan of treatment and plan of care.
• Observes, records and reports reactions to treatment and changes in client condition to appropriate personnel/physician.
• Provides ongoing instruction to client or family in the plan of treatment.
• Participates in education programs, quality assessment activities, nurse meetings and other staff meetings as required.
• Work schedule: Monday through Friday 8am to 5pm with weekend rotation.

As a valued LPN at Phoenix Home Health, you will serve as the vital link between our patients and their medical providers. We are looking for someone with exceptional skills in physical assessment and communication, enabling you to execute tailored care plans based on physician orders while incorporating insights from initial assessments. You will collaborate closely with fellow nurses, Physical Therapists, Speech Therapists, and Occupational Therapists. Responsibilities

• Provides nursing care in accordance with physician's plan of treatment, individual plan of care, as authorized by client's payer source
• Observes, records and report's reaction to treatment and any changes in client's condition to appropriate personnel and/or physician
• Provides ongoing instruction of client or family in the plan of treatment
• Participates in education programs, quality assessment activities, nurse's meetings and other staff meetings as required
• Monday through Friday 8am to 5pm with every 4 to 6 weeks, weekend rotation

• Missouri or Compact Registered Nursing or Licensed Practical Nursing License
