Phoenix Home Care & Hospice in Springfield, MO is seeking a Licensed Practical Nurse (LPN) for home health care, Monday through Friday days. Enjoy a 5–day workweek with a patient‑centered focus and the flexibility to avoid charting at home.

In this role you’ll perform skilled nursing visits, assess patients, educate families, and collaborate with an interdisciplinary team. AI-assisted charting supports your documentation during visits, and you can earn $60,000–$75,000 with a $2,500 sign‑on bonus.

Qualifications

• Current Missouri or Compact LPN license.
• Excellent assessment and communication skills.
• Strong organizational and time management abilities.

• Valid driver's license and reliable transportation.
• Home health experience a plus; eager to learn.

Responsibilities

• Deliver skilled nursing care in patients' homes according to physician orders and care plans.
• Perform comprehensive patient assessments and monitor changes in condition.
• Educate patients and families to promote confidence and independence at home.
• Collaborate with RNs, therapists, physicians and the interdisciplinary care team.
• Maintain accurate documentation using AI-supported charting.
• Participate in quality improvement initiatives and ongoing professional development.

What You'll Do

• Perform comprehensive patient assessments and monitor changes in condition.
• Educate patients and families to promote confidence and independence at home.
• Collaborate with RNs, Physical Therapists, Occupational Therapists, Speech Therapists, physicians, and the interdisciplinary care team.
• Maintain accurate documentation using our AI-supported charting technology.
• Participate in quality improvement initiatives and ongoing professional development.

Schedule: Monday through Friday, 8:00 AM to 5:00 PM, with a weekend rotation approximately every 4 to 6 weeks.
