Job summary

Adoration Health is seeking a Licensed Practical Nurse for our Home Health team in Clanton, AL. You will provide 1:1 patient care under the plan of care, administer medications, monitor conditions, and coordinate with family and physicians to support independence and dignity.

This full-time, day-shift role covers Autauga, Elmore, and nearby areas with flexible scheduling, a supportive team, and opportunities for growth.

Qualifications

• LPN/LPT/LVN degree with current unrestricted license in state of practice
• Valid driver's license with proof of car insurance
• CPR certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred

Responsibilities

• Providing 1:1 care per client’s plan of care while maintaining client’s dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per plan of care and assessments of results
• Documenting nursing actions and communication with family, case manager, and physicians; updating progress and outcomes
• Coordinating or ordering supplies, medications, and feedings to ensure adequate on-hand stock
• Ensuring all patient needs are met per physician instructions

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
