Adoration Health is seeking a Licensed Practical Nurse for Home Health in Clanton, AL. The role provides 1:1 patient care in clients\' homes with a schedule of 8AM-5PM, full-time days.

You will work within a supportive team, offering independence with team backing and growth opportunities with a growing home health organization. The ideal candidate holds an LPN/LPT/LVN license, a valid driver\'s license, CPR certification, and has 1+ years of nursing experience, preferably in home health.

Qualifications

• LPN/LPT/LVN degree with current unrestricted license in state of practice and residence.
• Valid driver\'s license, driving record, and proof of car insurance.
• CPR certification required.
• 1+ years nursing experience preferred.
• Home Health care experience preferred.

Responsibilities

• Provide 1:1 client care following the plan of care while maintaining dignity and independence.
• Administer medications, feeding, oxygen, ostomy care as needed per plan of care and assessments.
• Document nursing actions, communicate with family, case manager, and physicians; report changes.
• Coordinate or order supplies, medications, and feedings to ensure adequate stock.
• Ensure patient needs are met per physician instructions.

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
