Job summary

Adoration Health is seeking a Licensed Practical Nurse for home health in Alabama. The role involves providing 1:1 patient care per plan of care, medication administration, and ongoing documentation for coordinated care with physicians and families.

Requirements include an LPN/LPT/LVN degree with an active license, CPR certification, and 1+ year of nursing experience, preferably in home health. Travel to covered areas is expected."

Qualifications

• LPN/LPT/LVN degree with current unrestricted license in the state of practice.
• Valid driver's license with acceptable driving record and proof of auto insurance.
• CPR certification.
• 1+ years nursing experience; Home Health care experience preferred.

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence.
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results.
• Documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs.
• Coordinating or ordering supplies, medications, and feedings as needed to ensure adequate amounts are always on hand.
• Ensuring all patient needs are being met in accordance with the instructions of the physician.

• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
