Job summary

Adoration Health in Hamilton, AL is seeking a dedicated Home Health LPN to provide 1:1 patient care in the home setting. You will administer medications, monitor patient status, document care activities, and communicate with families and the care team.

This full-time role offers independent work with team support and opportunities for growth. Ideal candidates hold an LPN/LPT/LVN license, CPR certification, and have 1+ years of nursing experience, preferably in home health.

Qualifications

• LPN/LPT/LVN license with current unrestricted registration in the state of practice
• Valid driver's license with acceptable driving record and proof of insurance
• CPR certification
• 1+ years of nursing experience; home health care preferred

Responsibilities

• Provide 1:1 home health nursing care following the patient's plan of care
• Administer medications, feedings, oxygen and ostomy care as needed
• Document nursing actions and communicate with family, case manager, and physicians
• Coordinate supplies and medications to ensure adequate stock
• Ensure patient needs are met per physician instructions

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home H ealth care experience preferred
