Job summary

Adoration Health in Clanton, AL is looking for a dedicated Home Health LPN to join our team. You will provide 1:1 client care, administer medications, and document care under the client’s plan of care, while upholding dignity and independence.

This full-time day role serves Autauga, Elmore, and South Chilton with strong team support and opportunities for advancement. A current LPN license, CPR, and a valid driver’s license are required, with 1+ year of nursing experience preferred.

Qualifications

• Minimum 1+ year nursing experience preferred.
• CPR professional certification required.
• Valid driver's license with proof of insurance.

Responsibilities

• Provide 1:1 care to clients following the plan of care.
• Administer medications, feedings, oxygen, and ostomy care as needed.
• Document nursing actions and communicate with family, case manager, and physicians.
• Coordinate supplies, medications, and feeds as needed to ensure availability.
• Ensure patient needs are met per physician instructions.

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home H ealth care experience preferred
