Job summary

Aleca Home Health is looking for a Licensed Practical Nurse to deliver high-quality, patient-centered nursing care in patients’ homes in Mesa, AZ. You’ll use the nursing process to ensure continuity of care and optimum outcomes, supporting families and caregivers along the way.

Qualifications

• Current, unencumbered LPN/LVN license required or Compact State license.
• A minimum of 2 years LPN/LVN experience required.
• Able to educate patients and family on multiple disease processes and have good medication knowledge and wound care experience.

Responsibilities

• Complete and scan required record keeping forms/charts upon guest’s admission, transfer, and/or discharge.
• Chart nurses’ notes reflecting care provided and patient response.
• Report discrepancies concerning physician orders, diet changes, or charting errors to the Director of Nursing.
• Ensure documentation for unauthorized discharges is entered in the patient’s medical record per procedures.
