Job summary

Aleca Home Health is seeking a Master Social Worker to assess psychosocial status of patients and families, coordinating with the interdisciplinary team to support care plans.

The role involves counseling, evaluating family dynamics, caregiver support, and serving as liaison to community agencies to ensure seamless homecare. A current social work license and driving is required. Flexible schedule offered.

Qualifications

• Master’s or doctoral degree from a CSWE-accredited school of social work or a related bachelor’s degree in social work, psychology, sociology, or related field.
• Currently licensed to practice in the states where the role is performed.
• Valid driver’s license with a safe driving record and appropriate auto insurance.

Responsibilities

• Understands the goals of the plan of treatment and provides services accordingly.
• Employs counseling, behavioral and educational techniques to help clients cope with illness, injury, or disability.
• Counsels patients and families/caregivers regarding stress and coping difficulties.
• Serves as liaison between patients/families and community agencies; coordinates homecare with the clinical team.
• Ensures seamless homecare services through direct oversight and coordination of care.

Responsibilities

• Employs a variety of brief counseling, behavioral and educational techniques to assist clients to cope with an illness, injury and/or disability.
• Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, and plans intervention based on evaluation findings.
• Counsels patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties.
• Serves as a liaison between patients and families/caregivers and community agencies. Ensures seamless homecare services by providing direct oversight and coordination of care with other clinical service providers.

Qualifications

• A master’s or doctoral degree from a school of social work accredited by the Council on Social Work Education or has a baccalaureate degree in social work, psychology, sociology or other field related to social work.
• Currently licensed in the state(s) in which practicing.
• Valid driver's license with maintenance of safe driving record and an automobile that is insured in accordance with state requirements.
