Job summary

Four Oaks is seeking an Experienced LPN to coordinate and provide nursing care for youth in our Cedar Rapids programs. You’ll work with a team to safeguard health, safety, and success while building lasting relationships with the youths we serve.

You will administer medications, triage health concerns, and coordinate medical and behavioral health services. This role emphasizes advocacy, education, and careful documentation, with a meaningful on-call component.

Qualifications

• Active Iowa LPN license in good standing.
• Prior experience working with children preferred.
• Ability to work on an on-call rotational schedule.

Responsibilities

• Administer medications safely and accurately.
• Get to know the youths and build relationships based on trust.
• Assess health concerns and determine when additional medical care is needed.
• Provide medical triage and coordinate appropriate interventions.
• Coordinate medical, psychiatric, and psychological appointments and services.
• Manage physician orders, appointments, medication records, and documentation.
• Oversee medication management procedures and proper storage.
• Maintain confidential medical records and provide TB testing.
• Educate youth and staff on health topics; provide CPR/First Aid training.
• Review medication errors and participate in quality improvement.
• Report health, safety, and public health concerns; nursing oversight on safety protocols.
• Participate in periodic on-call needs, including daytime and overnight shifts.

What Does Nursing at Four Oaks Look Like?

As an LPN, you'll help coordinate and provide nursing care for youth in our programs while working alongside a team of professionals committed to their health, safety, and success.

You’ll:

• Administer medications safely and accurately.

• Maintain accurate and confidential medical records.
• Provide TB testing within established guidelines.
• Help educate youth and staff on health and wellness topics.
• Provide training in areas such as CPR, First Aid, and health education.
• Review medication errors and participate in continuous quality improvement.
• Recognize and appropriately report health, safety, and public health concerns.
• Provide nursing oversight related to established safety and behavioral protocols.
• Participate in periodic on-call needs, including occasional daytime and overnight situations.
