Linden Woods Village is part of Pivotal Health Care and is seeking a dedicated LPN Charge Nurse to join the team in Gladstone, MO. The role focuses on planning, organizing, and delivering resident care in a boutique senior living community with multiple levels of care.

The LPN Chargen Nurse orients and supervises staff, with evening shifts (6pm-10pm) and opportunities for full-time or part-time schedules. A state license and CPR certification are required, with preferred 1–2 years of experience

Competenze

• LPN must be licensed in the applicable state and maintain licensure/CEU requirements.
• Graduate from an accredited school; CPR certification required.
• Preferably 1–2 years of nursing experience as a Charge Nurse in long-term care or skilled nursing facility.

Mansioni

• Assess and implement resident care based on nursing standards and policy.
• Make daily rounds to monitor condition and comfort of residents.
• Initiate procedures to protect residents from injury and infection.
• Assist DON/RN with developing and evaluating care plans.
• Encourage restorative care and participation in activities.
• Monitor staff compliance with nursing policies and assess competency.
• Communicate with physicians and ensure hand-off information between shifts.

Role:

We are seeking a dedicated LPN - Licensed Practical Nurse to join our team! The LPN Charge Nurse plans, organizes, and assists in providing direct and indirect resident care following established policies and procedures, standards of nursing practice and principles of nursing care. Orients, instructs and supervises personnel.

Responsibilities:

• Assess and implement interventions for resident care based on standards of nursing practice and principles of nursing care, in accordance with Federal, State, and Company requirements.
• Make daily rounds to see all residents to ascertain condition and comfort.
• Initiate procedures to protect residents from injury and infection.
• Assist DON and/or RN with developing, implementing, and evaluating plans of care to include, but not be limited to, recognition of spiritual, emotional, psychosocial, and functional factors.
• Encourage and assist with restorative care in self-care and group activities.
• Monitor nursing personnel compliance with nursing policies and procedures and competency of clinical skills, evaluating nursing staff job performance with direction from DON.
• Communicate with the Physician as indicated by resident needs or status and assure follow-through; maintain hand-off communications with oncoming shift regarding resident care needs.
• Oversees direct care staff and ensures staff complete tasks and cares as assigned.
• Speak, read and write the English language in an understandable manner.
• Ability to apply principles of basic general math.

Qualifications:

• The LPN Charge Nurse must be a graduate from an accredited school, be currently licensed in the applicable state without encumbrance, be responsible for maintaining professional licensure and CEU requirements.
• Preferably 1-2 years of previous nursing experience as a Charge Nurse in Long Term Care and/or a Skilled Nursing Facility.
• Must be currently CPR certified.
