Job summary

Pivotal Health Care is seeking an LPN Charge Nurse to join Prairie Vista Village in Altoona, IA. The role plans, organizes and supervises direct and indirect resident care in a 5-star senior living community.

You will orient and supervise staff, ensure quality nursing care, and work with the DON/RN on care plans. Evening shifts are available, with full-time and part-time options to fit 30+ and 29 hours weekly schedules.

Qualifications

• The LPN Charge Nurse must be licensed in IA and maintain licensure and CEU requirements.
• CPR certification is required; prior experience as a Charge Nurse in long term care is preferred.
• Experience in a senior living or long-term care setting is a plus.

Responsibilities

• Assess and intervene for resident care following nursing standards and laws.
• Make daily rounds to monitor resident condition and comfort.
• Protect residents from injury and infection through proper procedures.
• Assist DON/RN with care plans, including spiritual, emotional, and functional factors.
• Encourage restorative care in self-care and group activities.
• Supervise nursing staff and ensure policy compliance and skills competency.
• Coordinate with physicians and ensure smooth hand-offs between shifts.
• Oversee direct care staff and ensure tasks are completed.
• Communicate clearly in English with the team and residents.

Role

We are seeking a dedicated LPN - Licensed Practical Nurse to join our team! The LPN Charge Nurse plans, organizes, and assists in providing direct and indirect resident care following established policies and procedures, standards of nursing practice and principles of nursing care. Orients, instructs and supervises personnel.

Responsibilities

• Assess and implement interventions for resident care based on standards of nursing practice and principles of nursing care, in accordance with Federal, State, and Company requirements.
• Make daily rounds to see all residents to ascertain condition and comfort.
• Initiate procedures to protect residents from injury and infection.
• Assist DON and/or RN with developing, implementing, and evaluating plans of care to include, but not be limited to, recognition of spiritual, emotional, psychosocial, and functional factors.
• Encourage and assist with restorative care in self-care and group activities.
• Monitor nursing personnel compliance with nursing policies and procedures and competency of clinical skills, evaluating nursing staff job performance with direction from DON.
• Communicate with the Physician as indicated by resident needs or status and assure follow-through; maintain hand-off communications with oncoming shift regarding resident care needs.
• Oversees direct care staff and ensures staff complete tasks and cares as assigned.
• Speak, read and write the English language in an understandable manner.
• Ability to apply principles of basic general math.

Qualifications

Qualifications: Qualifications:

• The LPN Charge Nurse must be a graduate from an accredited school, be currently licensed in the applicable state without encumbrance, be responsible for maintaining professional licensure and CEU requirements.
• Preferably 1-2 years of previous nursing experience as a Charge Nurse in Long Term Care and/or a Skilled Nursing Facility.
• Must be currently CPR certified.
